Peel CAS provides access to personal information and disclosure pursuant to Part X of the Child, Youth and Family Services Act and the Access to Adoption Records Act. All requests for access to and/or disclosure of information require copies of two valid government issued photo identification.
NOTE: Personal information may include your date of birth, contact information, records of meetings with you and/or your family, the services you or your child received, the programs you or your child attended, details of physical and mental health, medical, psychological or psychiatric reports, school information, financial information, employment history, allegations or findings of child maltreatment, court documentation, police interventions, criminal history, the information that you reported/provided contained in the record, your or your child’s views or opinions, the views and opinions of others about you or your child and information about you or your child’s race, ancestry, place of origin, colour, ethnic origin, citizenship, family diversity, disability, creed, religion, age, sex, sexual orientation, gender identity, gender expression, cultural or linguistic needs, marital or family status.

The written consent of any person over the age of 16 years is required to release their information to you, along with copies of their two valid government issued photo identification. Please click here to fill out the Disclosure to a Third Party Consent Form.
A person with custody of a child under the age of 16 may request personal information of that child, subject to some exceptions. In some circumstances, you may be asked to provide documents confirming your authority to make the request in place of the child.
For copies of birth registration and adoption orders for all adoptees, you must apply to the Ministry of Community and Social Services at Service Ontario or toll free at 1-800-461-2156.
If you were adopted or are related to someone who was adopted through Peel CAS and you would like to request non-identifying information, please fill out the Disclosure Request form for Non-identifying Adoption Information. If you are a descendant of an adult adoptee who is deceased, a copy of the death certificate will be required.
